This gentleman had undergone 5 prior explorations for recurrent infection and inflammation following a palm thorn puncture injury two years ago. In this case, the scar and fibrotic dorsal metacarpal head skin was excised in addition to a radical joint debridement, and an island first dorsal metacarpal artery flap was used to relieve tension and provide well vascularized cover over the extensor tendon. |
